Satisfaction Questionnaire
The following
is a generic participant satisfaction questionnaire that you
may want to use
as a template to gather information to evaluate your mental
health promotion project.
We welcome
your feedback on how (the name of your project) is doing
to promote mental health
in the community. You don’t need to include your name. Your
feedback will be confidential.
For each
item, please circle the number that best indicates your
satisfaction with a particular aspect of the initiative.
The results of this survey will be compiled by the Project’s
steering committee, and will be used to help us determine
how effective the project has been so far, and what kind
of changes we should make to improve the project for the
future. Please provide additional comments if you wish.
